What Depression Is and Why Willpower Alone Cannot Fix It

Depression refers to a persistent low mood, loss of interest, and drop in energy lasting at least two weeks, which changes how you sleep, eat, concentrate, and relate to people. Willpower has limited traction on it because depression dampens the systems you would use to push through.

Here is the version we hear most often in a first session. You are still getting to work. The emails still go out. Nobody in your life has any idea. Then you get home, the evening disappears, and you cannot explain why loading the dishwasher feels like a mountain when you managed a full day of meetings.

Depression narrows things. Interest goes first, and the things you dropped were the ones refilling you. Sleep goes sideways, either too little or far too much. Concentration thins out. The internal commentary gets meaner and, because depression also removes your ability to argue with it, it starts sounding like plain fact rather than a symptom.

Statistics Canada found that 7.6% of people in Canada aged 15 and older met the criteria for a major depressive episode in the year before the 2022 survey, up from 4.7% a decade earlier. Nearly a third of people who met criteria for a mood or anxiety disorder reported their need for counselling was partly or fully unmet. A lot of people are carrying this without support.

The Evidence-Based Therapy Approaches That Treat Depression Effectively

Cognitive Behavioural Therapy and behavioural activation carry the strongest research support for depression, and Canadian clinical guidelines list both as first-line treatments. Acceptance and Commitment Therapy and Compassion Focused Therapy add something the research supports for specific pieces of the picture, and a good treatment plan usually borrows from more than one.   • Cognitive Behavioural Therapy works on the loop between what you think, how you feel, and what you then do or avoid. You learn to catch a thought like “I ruined that meeting,” check it against the evidence, and land somewhere more accurate. It is structured and usually involves practising something between sessions.

  • Behavioural activation is the behavioural half of CBT and it is deceptively powerful here. You rebuild activity first and let the mood follow, rather than waiting to feel like it. Small and specific beats ambitious: a ten-minute walk on Tuesday, not a new gym membership.

  • Acceptance and Commitment Therapy helps you stop wrestling every difficult thought and start choosing actions based on what matters to you. For people who have spent years trying to think their way out of depression, putting that fight down is a relief.

  • Compassion Focused Therapy addresses the self-criticism that almost always rides along with depression. When the loudest voice in your head has been contemptuous for a decade, changing your relationship with it is part of the treatment.

  • EMDR and Accelerated Resolution Therapy come in when the depression sits on top of unprocessed trauma or loss. Treating the mood without touching what is underneath it tends to produce short-lived gains.

Each of these is a tool rather than a package. Two people with the same diagnosis often need different work, which is why the treatment plan gets built with you.

What a Depression Treatment Plan Usually Includes

  • A clear picture of your sleep, appetite, energy, and concentration, so progress is measured against something real.

  • Behavioural targets small enough to actually happen this week, and work on the thinking patterns that keep the low mood self-confirming.

  • Attention to the self-criticism, which is usually the part clients apologise for even mentioning.

  • A plan for safety if you have been having thoughts of harming yourself.

  • Coordination with your physician if medication is part of the picture.

How Therapy and Medication Work Together When Both Are Needed

For moderate to severe depression, therapy and medication together tend to outperform either one on its own. A network meta-analysis of 101 trials with 11,910 patients found combined treatment more effective than psychotherapy alone and more effective than medication alone, with no significant difference between the two monotherapies.

That last part matters. Therapy on its own is a legitimate first-line treatment, and plenty of people recover without ever filling a prescription.

Medication earns its place when symptoms are severe enough that therapy cannot get a foothold. If you are sleeping four hours a night and cannot hold a thought long enough to finish it, medication can lift you far enough to engage with the work.

Psychologists in Alberta do not prescribe. If medication looks worth considering, that conversation happens with your family physician or a psychiatrist, and we will coordinate with them so you are not the one relaying information between two offices.

What the Early Weeks of Depression Therapy Often Feel Like

The first few weeks usually feel like effort before they feel like relief, and that is important to note so you do not read it as a sign that therapy is not working.

Session one is mostly history and orientation. Your therapist is working out how long this has been going on, what it looks like in your life, what you have already tried, and what you want back.

Weeks two through four are where the effort shows up. You are asked to do small things at the exact point when doing anything feels like too much. That is the hardest stretch, and it is where healing lives. If these tasks feel impossible, then you’ll explore the thoughts or emotions getting in the way and come up with a plan to address them.

What clients notice first is usually quiet and specific. A Tuesday afternoon that felt lighter. One task done without the whole day disappearing. Waking at six instead of four. Those small returns arrive well before the bigger shifts do.

If nothing has moved after eight to ten sessions, say that out loud to your therapist rather than quitting quietly. Sometimes the approach needs to change. Sometimes something underneath the depression has not been named yet.
